This case study presents a detailed CT analysis of a patient one year after square jaw and chin surgery, including masseter muscle reduction. We will objectively evaluate the surgical outcome by comparing CT scans taken pre-operatively, 3 months post-operatively, and 1 year post-operatively, highlighting interesting changes observed throughout the recovery process.
1. Pre-operative Analysis
The chin deviated 3.6mm to the left. The width of the zygomatic arch was slightly wider on the right side (72.47mm/71.65mm).

The pre-operative face had a predominantly square appearance, masking the chin asymmetry.
The mental protuberance was more developed on the left side.

mental protuberance Square jaw surgery and chin surgery were planned to achieve facial balance and a slimmer facial contour.
2. 3 Months Post-operative
A significant reduction in facial width was observed, with no noticeable asymmetry.

Interestingly, the midface appeared fuller despite only undergoing square jaw and chin surgery. This could be attributed to soft tissue redistribution or post-operative swelling.

3. 1 Year Post-operative
The fullness in the midface significantly decreased, likely due to the resolution of post-operative swelling.

The patient experienced a 0.9kg weight loss and a 1% decrease in body fat percentage between the 3-month and 1-year post-operative marks.
Increased thickness around the lips suggests changes in lip movement habits, possibly due to increased lip muscle use.
Despite weight loss, certain areas exhibited increased soft tissue thickness, likely related to soft tissue (muscle, buccal fat) redistribution following square jaw surgery.

4. 3 Months vs. 1 Year Post-operative
Comparison of the 3-month and 1-year post-operative CT scans revealed downward bone growth.

Soft tissue increased bilaterally by 1.78mm between the 3-month and 1-year post-operative periods.
Masseter muscle thickness increased by 2.25mm on the right and 2.65mm on the left compared to the 3-month post-operative measurements.

Midface swelling decreased by 1.53mm bilaterally after square jaw surgery.

The jawline definition became more pronounced.

5. Further Analysis and Discussion
The patient experienced a total weight loss of 3.4kg and a 2.4% reduction in body fat percentage from pre-operative to the 1-year post-operative mark.
Despite the 0.9kg weight loss and 1% body fat reduction between the 3-month and 1-year post-operative periods, the masseter muscles thickened. This highlights the impact of factors like teeth grinding and clenching habits on masseter muscle size.
Improving the habit of clenching teeth can enhance long-term facial slimming effects.
Adequate chewing exercises can promote brain activation, suggesting the importance of combining a healthy diet with regular chewing exercises
Conclusion
This case study provides a comprehensive CT analysis of changes observed one year after square jaw and chin surgery. Long-term follow-up observations are crucial for patient satisfaction and maintaining successful surgical outcomes. Continuous management and effort are necessary to achieve optimal results.
